Help - DMS3 v.9 on a PS2 v4 Motherboard

I have been searching and SEARCHING for days to find an installation guide for a DMS3 version 9 chip to stick into a PS2 (SPCH-30001 - v4)

The best site I could find is This One

And it does not have a version 4 motherboard install guide with a DMS3v9 chip....

and the DMS3.com site is DOWN.. found a post saying it was down in 2003.. with no word on it coming back up... and I know they had EVERY single guide needed..

Anyone able to help?
(BTW - my PS2 DVD lens crapped out and I want to boot from a memstick and play my backups off of an external hard drive)
